BONE & TISSUE MASTERCLASS

by Prof. Howard Gluckmann

Lecture 1:

The Symbiotic relationship between bone and soft tissue

Bone and soft tissue loss are a reality in implantology. Most cases require some form of tissue graft or another and there are very few that have the requisite structure in place for ideal treatment outcomes. Long term aesthetic stability and health of an implant is dependent on both bone and soft tissue. The symbiotic relationship between the two is crucial for long term success. This lecture series will go through the different protocols and share insight into achieving predictable results with both bone and soft tissue grafting. We will share hints and tips accumulated over years if practice including failures and successes with different treatment options.

Lecture 2:

Partial Extraction therapies

The loss of teeth will lead to resorbtion of the buccal plate with its resultant need for extensive augmentative procedures (both bone and soft tissue) as well as the risk of poor long term aesthetic stability. Partial Extraction Therapies (PET) are techniques that allow us to maintain the buccal bone plate and hence prevent the collapse of the alveolar bone creating a platform for ideal soft tissue and bone which is stable in the long term. This is a greater problem in the aesthetic areas. This lecture will take you through the different options with regards to immediate implant placement as well as the most cutting edge PET in a step by step fashion.

Lecture 3:

The management of single implant sites in the aesthetic zone from preservation to rehabilitation

The aesthetic zone presents several very specific problems which are exacerbated following tooth loss. The loss of bone and soft issues, as well as the natural gingival contour, makes replacing the teeth with implants a huge challenge. The attention to detail regarding new bone growth followed by soft tissue grafting and prosthetic soft tissue development are key to achieving an excellent aesthetic result that is stable over the long term. There are a number of key parameters that need to be followed to achieve the predictability of our results. The decision-making tree is paramount and differs from patient to patient, and all of these factors will be brought together in an algorithm that allows one to choose the best treatment option for the patient. From Digital Smile design to choose of bone graft material to timing of soft tissue surgery. The lecture will share more than 25 years of failure and success, but with a constant striving for improvement and excellence.

Prof Howard Gluckman is an internationally renowned dental practitioner, implantologist, author, and lecturer. In addition to running his full-time private practice in Cape Town, Howard is also the co-founder and director of the Implant and Aesthetic Academy (IAA) and an Adjunct Assistant Professor in the Department of Periodontics at the University of Pennsylvania.

After completing his dental training at the University of Witwatersrand in 1990, Howard spent a few years in general practice. He then took on a four-year degree in Oral Medicine and Periodontics at the University of Stellenbosch, which he completed with Distinction (Cum Laude). Eager to shape the industry and help others expand their knowledge, Howard became instrumental in developing the University of Stellenbosch and the University of the Western Cape’s postgraduate Implantology Diploma. Not one to rest on his laurels, Howard completed his PhD. Summa Cum Laude at the University of Szeged in Hungary, supervised by Professor Katalin Nagy.

Howard specialises in Immediate Implant Placement/Immediate Load, Soft Tissue Aesthetics, and Periodontal Plastic Surgery. He also has a special interest in autogenous bone augmentation, especially bone harvested from the palate (a new technique he published in 2015). Howard also specialises in three-dimensional bone augmentation.

In addition, Howard is an expert in and thought-leader on Partial Extraction Therapy. His expertise includes Socket Shield, Pontic Shield (a procedure he developed and published in 2016), and Submerged Root Technique (the topic for which he achieved his PhD).

Howard has achieved numerous career milestones, including serving as the South African Society of Periodontics (SASPIO) secretary, Past President of the South African Society for Dental Implantology, Associate of the College of Medicine and Dentistry of South Africa, and Diplomat of the International Congress of Oral Implantologists. In addition, Howard is a member of several committees, including the DentalXP scientific board and the Southern African Association of Osseointegration (SAAO) board. He is also a member of the South African Dental Journal, Indian Journal of Prosthodontics, and Hungarian Journal of Dentistry editorial boards.

Howard is a prolific and influential author. His work is found in various implantology textbooks and numerous peer-reviewed scientific journals, and book chapters.
